Community health services are well known for delivering innovative and responsive models of care in collaboration and partnership with other services who support the social model of health.

We can leverage what already exists by integrating services and increasing access within the community, which in turn will alleviate hospital demand. By joining the Community Health First campaign we can give a voice to our communities and influence investment in the health and wellbeing of all Victorians.
